Player Waiver
& Release

I understand that participation in soccer training and related activities with Praxis Soccer LLC involves physical activity and inherent risks, including but not limited to injuries, collisions, environmental conditions, and other risks that may result in serious injury or death. I voluntarily choose to participate or allow my child to participate.

I knowingly and voluntarily assume full responsibility for all risks associated with participation in Praxis Soccer LLC activities.

I release, waive, and discharge Praxis Soccer LLC, its owners, coaches, staff, contractors, affiliates, and facility partners from any and all liability, claims, or damages arising from participation, including those resulting from ordinary negligence.

I agree to indemnify and hold harmless Praxis Soccer LLC from any claims or damages resulting from participation in activities.

I certify that the participant is physically able to participate. I authorize emergency medical treatment if necessary and accept responsibility for any related costs.

I grant Praxis Soccer LLC permission to use photos, videos, and recordings of the participant for marketing, social media, and educational purposes without compensation.

I have read and understand this agreement and acknowledge that I am waiving certain legal rights, including the right to sue.
